The Role:
Here at Sedgemoor Dementia Support Hub, we have an exciting opportunity to recruit to several Reablement Assistants (Care Assistants) posts working within Liverpool City Councils in house care delivery directorate. These particular posts will be based within Sedgemoor Dementia Support Hub, although there may be a requirement to work anywhere within City boundary.
We want to support people living with dementia to live well for longer, stay at home for longer, and for them and their families to feel well supported. We work closely with Merseycare Community Mental Health Team (CMHT) and other dementia related partners. You can really make a difference to somebody’s life by bringing a smile, energy & passionate approach to care.
The role of a Reablement Assistant (Care Assistant) in the support hub is to work on the front-line, providing person centred care and support for people who are living with dementia. As a Reablement Assistant (Care Assistant) you will be required to use safe moving and handling procedures including the use of assessed equipment.
As a Reablement Assistant (Care Assistant) will provide dementia appropriate activities for people to engage in throughout the day. Supporting people through times of distress & anxiety by using an empathetic and a personalised approach.
As a Reablement Assistant (Care Assistant) will have responsibility to be the important link person to the person accessing our service and their families. Being that listening ear, signposting to other services, working closely with the team organisers to ensure care plans and risk assessments are reflective of peoples changing needs.
