The Role:
- We are looking to recruit for a permanent appointment to a Senior Practitioner post within the North Locality of Liverpool. The successful candidate will be part of the Stanley Team based in the Picton Office.
- This is a great opportunity for the right candidate to gain experience in a leadership role supporting a newly established team
- The Senior Practitioner will support adults in several community settings, some of which include people in their own home, care homes, extra care environments, supported living facilities and hostels. You will ensure a strengths-based approach is implemented in the completion Care Act assessments, reviews and carry out s42 (safeguarding) enquiries
- You as the Senior Practitioner will co-produce support plans that will increase inclusivity, collaboration, and innovation, leading to more sustainable solutions so that people achieved their outcomes
- The Senior Practitioner will have responsibility for a small number of complex cases and will support practitioners with case management. Senior Practitioner responsibilities include supporting the Team Manager in supervising social care assessors and ASYE Social Workers and strengthening working relationships with health and partner agencies through strengths based and collaborative practice
