Join Our Hospital Discharge Team – Make a Real Difference Every Day 

Are you looking to develop your management skills while playing a vital role in patient care? We’re seeking motivated professionals to join our dynamic Hospital Discharge Team. As a key part of the multidisciplinary team, you’ll help ensure safe, timely, and person-centred discharges by assessing needs, coordinating services, and advocating for patients and their families. 

This vital service focuses on enabling safe, timely, and person-centred hospital discharges by providing early intervention, short-term support, and robust assessments. The team works collaboratively with Nurses, Therapists and health colleagues to deliver a strengths-based response that promotes independence and reduces reliance on long-term care.

The Role: 

  • A Senior Practitioner will support complex discharge planning and commission services such as domiciliary care, respite, or placements as required
  • A Senior Practitioner will supervise Social Care Assessors and Social Workers
  • A Senior Practitioner will support with discharge meetings
  • A Senior Practitioner will participate in daily MDTs to review and manage complex discharges with a collaborative approach
  • A Senior Practitioner will ensure smooth transitions to Locality Teams for individuals requiring long-term support
  • A Senior Practitioner is a dynamic and rewarding role where you can directly influence better health and social care outcomes for adults across Liverpool
  • A Senior Practitioner will  make an impact where it matters most. Join our Hospital Discharge Team and help people return home safely, with dignity and the right support in place
