The Role:
Adult Access is the front door of Adult Social Care & Health which enables a clear direction for enquires, resource, demand management, and accountability.
Adult Access has three areas of operations to enable the delivery of the wider Adult Social Care Target Operating Model. These three functions are:
- Wellbeing Advisor
- Multi-Disciplinary Team (MDT)
- Safeguarding Triage
All three functions will focus on a seamless person-centred, strength-based approach from first contact. In practice, Wellbeing Advisors, Social Workers, Social Care Assessors and Occupational Therapy will work together as a team. The aim being to prevent, reduce and delay people needing formal and longer-term care, and to provide short term reablement support, where required, to enable people’s independence.
Your role will be as a Social Worker within the fast-paced Multi-Disciplinary Team.
- Your role as a Social Worker will be to assess and where necessary, provide reablement or short-term support to individuals to promote their independence. Where long term intervention is required, a robust assessment will be completed to enable a smooth transition to the Locality Teams for on-going intervention
- Social Worker’s embed a strength-based approach to assessment with outcomes where people have wider choice and control over their well-being
- Your role as Social Worker will involve working with a wide range of cases for all adults from the age of 18
- Within your role as a Social Worker, you will be required to complete Reablement Assessment, Your Needs Assessment, Risk Assessment, Mental Capacity and Best Interest assessment, completing Reviews, as well as any other relevant assessment in relation to a person’s needs
- As the Social Worker for Liverpool City Council, you will commission services such as Domiciliary Care packages, Day Centre placement, Carers support such as Respite and support for complex cases etc
- As the Social Worker you will be responsible for social work provision including managing own caseload whilst managing the reputation of the directorate and council and complying with national and service procedures and guidance
- Within the role, our Social Workers will be expected to travel to complete home visits for the purpose of assessments and or welfare visits, and or attend other Council Buildings across the City
- Your role as the Social Worker within the Multi-Disciplinary Team will also include attendance of the daily huddle to discuss any complex cases with an MDT approach
